Success Stories

  • Barnard Consulting partnered with Fighting Back Santa Maria Valley (FBSMV) to develop a comprehensive Rapid Rehousing model, ensuring alignment with best practices in housing-first strategies.

    Our team:

    • Designed housing support services that address the complex needs of clients, integrating case management, rental assistance, and landlord engagement.

    • Provided guidance on CalAIM service integration, helping FBSMV leverage Medi-Cal funding to expand behavioral health and supportive services.

    • Strengthened the organization’s capacity to deliver long-term housing solutions, improving outcomes for individuals transitioning from homelessness.

    Through this collaboration, FBSMV successfully expanded its housing services, providing critical support for individuals and families seeking stable housing.

  • As a technical assistance provider, Barnard Consulting has played a key role in helping Good Samaritan Shelter (GSS) grow and sustain its housing and support programs. Our team:

    • Assisted in securing grant funding to expand emergency shelter, transitional housing, and permanent supportive housing programs.

    • Supported program development, ensuring services remained responsive to the evolving needs of the unhoused community.

    • Provided compliance and reporting guidance, helping GSS build organizational capacity and sustainability.

    Our work has helped GSS expand its impact, reaching more individuals and families with the housing and support they need to rebuild their lives.

  • Barnard Consulting provided technical assistance to justice-involved providers in Santa Barbara County, supporting them in integrating CalAIM services to improve health outcomes through coordinated care.

    As a CenCal Health Steering Committee member, Sylvia Barnard led efforts to:

    • Build stronger partnerships between justice and health systems to enhance care coordination.

    • Help providers navigate CalAIM services, ensuring justice-involved individuals receive comprehensive support.

    • Address systemic barriers in rural and underserved communities, improving service accessibility.

    By aligning healthcare, behavioral health, and housing services, this project contributed to better outcomes for individuals transitioning from the justice system.

Presentations & Conferences

  • In May 2024, Sylvia Barnard presented at the Center for Healthcare Strategies, highlighting the importance of Day Habilitation programs. These programs provide structured activities that help individuals with developmental disabilities build life skills, engage in meaningful work, and gain independence.

    Sylvia’s presentation:

    • Showcased Good Samaritan Shelter’s Day Habilitation programs, illustrating how they align with CalAIM's broader health integration goals.

    • Provided an overview of funding pathways, including Medicaid and state disability services, to support program sustainability.

    • Emphasized how integrating community supports strengthens services and improves long-term outcomes.

    This session reinforced the value of Day Habilitation programs in helping individuals achieve greater self-sufficiency and community inclusion.

  • In February 2024, Alexis Nshamamba presented at the National Alliance to End Homelessness conference, leading a session on braided funding strategies to support housing and services for aging populations.

    Alexis provided key insights on:

    • How nonprofits can combine public and private funding sources to create sustainable housing and service programs.

    • Best practices for securing funding for aging individuals experiencing homelessness or health disparities.

    • Strategies for leveraging resources in rural areas, ensuring equity in access to housing and healthcare.

    This project helped organizations strengthen their financial sustainability and expand support services for aging populations in need.

  • At the June 2023 National Alliance to End Homelessness conference, Alexis Nshamamba led a presentation on Housing Retention Multidisciplinary Teams (MDTs). These teams provide wraparound services to individuals at risk of losing stable housing, using an integrated, client-centered approach.

    Through this initiative, Barnard Consulting:

    • Supported the development of cross-sector MDTs, bringing together case managers, social workers, and healthcare providers.

    • Helped organizations build capacity to prevent homelessness and improve housing stability.

    • Addressed the unique needs of high-risk populations, including individuals with histories of trauma, chronic health conditions, and justice involvement.

    This project demonstrated how collaborative service models can effectively reduce homelessness and support long-term stability.
